Join Our Team as a Chemical Dependency Program Case Manger at Harlem East Life Plan (HELP)

Role Overview:
We are seeking an experienced Chemical Dependency Program (CDP) Case Manager to join our dedicated team. In this role, you will provide compassionate, professional care to help our clients stabilize their lives, access-needed resources, and build a healthier future.
Key Responsibilities:
- Provide counseling to individuals and families in recovery, including psychosocial assessments and coordination with medical and support services.
- Manage a focused caseload to ensure personalized, high-quality care.
- Provide support and referrals for housing, employment, benefits, childcare, legal issues, and wellness.
- Create and maintain treatment plans aligned with Federal, State, and OASAS standards.
- Act as a liaison between clients and internal agency resources.
- Maintain accurate, compliant, and audit-ready client records.
- Participate in treatment team meetings and case conferences.
Qualifications:
- Bachelor’s degree in Arts or Science or 5+ years of experience in a human services agency.
- Qualified Health Professional under OASAS regulations (e.g., Licensed Master Social Worker, Licensed Mental Health Counselor, CASAC, CASAC-T).
- Strong organizational skills and attention to detail.
- Compassion, empathy, and a genuine commitment to client well-being.
- Ability to work both independently and as part of a team.
- Understanding of ethical guidelines and confidentiality standards.
